Division 1
Lifting & Rigging Inspection
Using lifting accessories with missing or illegible WLL tags, or "homemade" modified equipment without professional engineering certification.
A contractor's entire batch of web slings was flagged by a safety auditor because capacity tags had worn off — causing an unexpected site shutdown.
An emergency site audit quarantined non-traceable slings. A physical check and manufacturing records review allowed compliant assets to be re-tagged and returned to service.
If the load tag is missing or illegible, it is legally non-compliant and a massive liability — regardless of how it looks.
Division 2
Non-Destructive Testing (NDT)
Requesting PT or MPI testing on metal parts heavily covered in paint, thick grease, or rust without proper surface prep.
A plant manager ordered an urgent MPI weld inspection on a crane runway without removing old industrial paint coatings first.
After proper sandblasting, a 40mm surface crack was immediately detected and repaired — preventing structural failure.
Surface cleaning is a critical scientific prerequisite. NDT cannot "see" through heavy contaminants or paint layers.
Division 3
Calibration Services
Assuming load cells and pressure gauges stay accurate just because the annual certificate hasn't expired, even in harsh environments.
A logistics firm noticed crane lift discrepancies yet assumed instruments were fine since they were within the calibration window.
A lab check revealed a 7% calibration drift caused by severe on-site physical impacts — invisible to the naked eye.
Environmental abuse overrides calendar dates. Any hard impact or strange reading demands immediate recalibration.
Division 4
Training & Assessment
Assuming a licensed crane operator automatically knows how to select, inspect, and balance a rigging load safely.
A load slipped mid-air on a construction site. The operator was licensed, but ground crew used incorrect sling angles.
A formal on-site Rigging & Slinging competency assessment covered center-of-gravity calculations and safe hitch configurations.
A crane is only as safe as the rigger on the ground. Ground crews need separate formal competency assessments.
